Use the templates below to request a release from your current upline. Ensure you are asking for a release from your top-of-hierarchy. If you don’t know who your top-of-hierarchy is, ask the carrier directly. The contact information for each carrier can be found here.

Note for agencies: when requesting a release, list all of your agents and their NPN numbers in your request to your current upline. Ask your current upline to list all of the agents on the release, along with your name and agency name.
