“Write in Reason for Appointment” with SOA if within 48 Hours Feature

When sending an SOA, if it is within 48 hours, you are prompted with an optional space to fill in the Reason for appointment.
When the agent opens the SOA, they will see the Reason for appointment space on the form.
When the agent downloads or prints the SOA, they will have the Reason for appointment visible as well.
